Section 1, Some Conventions in Mathematics , has some examples and exercises. Purple Math has a lesson at http://www.purplemaths.com/modules/evaluate.htm, called evaluation at, it also looks at function notation. There is more basic information at http://www.algebrahelp.com/lessons/simplifying/substitution/. There are some examples and exercises at http://www.mathstutor.com/Algebra.html but no answers to the exercises.

There is a Massey Universtiy Site that covers most topics well and is worth having a good look at. There are also some quizzes at http://webct-ce.massey.ac.nz/SCRIPT/tsamith1/scripts/serve_home that can help you identify problem areas.
